Percentage of active patients with a history of diabetes (self-reported or physician diagnosed) who received follow-up care (evaluation or prophylaxis or periodontal treatment procedure) in the practice during the measurement year Numerator: Patients who received evaluation or periodontal care D0120 or D0150 or D0180 or or D1110 or D1120 or D4346 or D4341 or D4342 or D4910 or D4240 or D4241 or D4260 or D4261 (treatment performed) during measurement period Denominator: Patients who have diabetes ICD codes or free text or self-reported medical history element prior to the end of the measurement year The denominator is a subset of the Initial Patient Population defined as patients with D0120 OR D0145 OR D0150 OR D0180 (treatment performed) within the 3 years prior to the start date of the measurement period and including the measurement period. Exclusions: [At any time prior to the start of the measurement period] Patients who are completely edentulous
